A new endobronchial tube for broncho-pleural fistula repair.

چکیده

IN anaesthesia for the surgery of pulmonary tuberculosis, bronchial blocking appears to be the exceptional rather than the usual procedure. This is, no doubt, largely due to the much improved preoperative treatment nowadays of patients with sputum. Moreover, there has not yet been published by those who practise bronchial blocking a convincing series to show that their cases have a lower collapse and spread rate than a series done under exactly similar conditions in the same hospital, but without the use of blockers. A decided advantage, of course, arises from doing a good deal of bronchial blocking, for when those cases come along which undoubtedly need it, one is wellpractised, and little tempted to try and do without it.

Turpentine-induced chemical pneumonitis with broncho-pleural fistula.

Turpentine is a volatile hydrocarbon used in polishes, solvents, paints and textile industry. When hydrocarbons are aspirated into the lung, they cause chemical pneumonitis, acute respiratory distress syndrome (ARDS), and rarely pneumatoceles and pneumothorax. We report a 20-year old boy with turpentine-induced chemical pneumonitis that evolved into a bronchopleural fistula. He was treated with...

Use of a modified endobronchial tube for mechanical ventilation of patients with bronchopleural fistula.

Mechanical ventilation in patients with bronchopleural fistula after lung resection is a major problem, as it causes increase of the air-leak, complicates the healing process and makes residual lung tissue ventilation difficult. We present two cases in which the use of a modified double lumen endobronchial tube improved ventilation and eliminated the fistula air-leak. We used a right-sided doub...

Fatal broncho-pulmonary artery fistula after lobectomy for lung cancer†

A broncho-pulmonary artery fistula is one of the most fatal complications of lung cancer surgery. This article discusses the case of a patient who died of massive hemoptysis after a left upper lobectomy. There were no previous signs of broncho-pleural fistula except for an obstinate dry cough and slightly elevated serum C-reactive protein levels after surgery. An autopsy revealed that a fistula...

Bronchoesophageal Fistula Stenting Using High-Frequency Jet Ventilation and Underwater Seal Gastrostomy Tube Drainage

Managing a patient scheduled for bronchoesophageal fistula repair is challenging for the anesthetist. If appropriate ventilation strategy is not employed, serious complications such as hypoxemia, gastric distension, and pulmonary aspiration can occur. We present the case of a 62-year-old man with a bronchoesophageal fistula in the left main stem bronchus requiring the insertion of a Y-shaped tr...
